Submit three (3) SIGNED letters of recommendation outlining and describing your EMS skills and teaching abilities.

Submit documentation (SIGNED Letter) to verify you have fifteen (15) hours of instruction experience within the last 12 months.

Submit documentation (SIGNED Letter) to verify that you have provided thirty (30) hours of actual patient care within the past year prior to endorsement (EMT, AEMT, EMT-IA, or Paramedic only).

You must go into the application section (emslicense.utah.gov) and start an application for your EMS Instructor.

** Registration and attendance of the course does not automatically award you the EMS Instructor endorsement.

Step 1:

To enroll, go to emslicense.utah.gov, select education, enter in the course number, and click on the course and register.

Step 2:

Go to applications and select the application you wish to start. Go through the application process, pay any applicable fees, and submit any needed documents.

Complete an EMS Instructor course and sign a contract.

Once the course is complete and verified, your endorsement will be approved.

In order to complete your endorsement, you will need to ensure you have completed your application, paid your $150.00 endorsement fee, provide 3 SIGNED letters of recommendation, and provide a SIGNED letter verifying 15 hours of EMS Instruction and 30 Patient care hours.
