EMS PROFESSIONAL BACKGROUND CHECK REQUIREMENT: DACS FORM MUST BE PRESENT FOR BCI FINGERPRINTING

Utah law requires the Bureau of Emergency Medical Services (BEMS) to exclude individuals from EMS licensure who may pose an unacceptable risk to public health and safety, as indicated by an applicant’s history. As part of this process, BEMS conducts a background screening on all EMS Professional license applicants, including an FBI background screening.

To prevent billing errors, lost fingerprints, processing delays, and repeat fingerprinting, it is critical that all students follow the required background screening steps exactly as outlined below.

*Current delays are being experienced in DACS form creation. As these forms are manually created by BEMS staff, the increase in new courses starting in January has substantially increased the processing queue. Please be patient as we are processing forms as quickly as possible.

Required Background Screening Process

After an applicant has submitted their EMS Professional application and paid all required fees:

  1. Print the LiveScan Authorization Form (DACS Form).

    • This form is located in the Documents area on the left-hand side of the applicant’s account at emslicense.utah.gov.
    • Navigate to My Account > Documents.
    • The file will be saved as DACS Form.
    • It is recommended that students use a computer to access and print the form.
  2. Review the DACS Form for accuracy.

    • Check for inaccuracies or any fields marked “unknown.”
    • If there are errors or the form is expired, the student must email ems@utah.gov for correction before fingerprinting.
  3. Find an approved LiveScan facility.

    • A list of approved LiveScan facilities in Utah is sent via email with notification of DACS form delivery. This list is available on the BEMS website.
    • Most LiveScan locations require an appointment.
  4. Students must take their printed DACS Form with them to the LiveScan appointment.

    • This is required.
    • Fingerprinting must not be completed without the physical DACS Form present.
    • Failure to bring the DACS Form may result in rejected or lost fingerprints, repeat fingerprinting, and potential fines or billing issues.
  5. Ensure the LiveScan facility enters the correct OAC/Agency Code.

    • This code is located in the Applicant Information section of the DACS Form.
    • When entered correctly, background results are reported directly to the Bureau of EMS.
    • There is no need to submit the signed form to BEMS separately.

Important Instructor and Course Coordinator Guidance

  • Students may continue in their course and complete testing while their background check is pending.
  • Delays in receiving DACS forms should not prevent course progression.
  • However, students must not be sent for fingerprinting until the DACS Form has been issued and printed.

Training programs are expected to clearly communicate this requirement and verify student compliance before directing students to any LiveScan location.
